If you are ordered to have ignition interlock, the process will be explained to you during intake by your case manager. Please bring your vehicle registration with you on the day of intake. (If you have more than one vehicle registered or titled in your name, in whole or in part, DMV requires you to install the device on each vehicle listed in their records. If you have a subsequent DUI, it is your responsibility to notify DMV of any vehicles not registered or titled in your name due to being sold, donated or otherwise disposed of.)

After your Intake appointment, you will either be referred to the treatment service provider of your choosing for an treatment assessment or assigned a ten week state-required education class. If you are assessed as requiring treatment, you will be assigned to the four week education classes. The intervention education classes cost $100. The cost of treatment varies and is assessed by the treatment provider. Some of the treatment providers accept private insurance.

Should you miss any of your classes without official documentation and approval your case manager, you will be charged a $25.00 rescheduling fee.

You may be randomly selected to provide a urine drug screen; the cost for this is $25.00. You will be given Lab forms to take to LabCorp. Payment are expected at the time of the screening.

Rescheduled appointments are $25.00 unless you provide sufficient notice and receive approval your case manager.

If you are required to be monitored by Peninsula ASAP for an extended period of time (usually three years), you will be required to attend scheduled case reviews to determine your progress. The charge for each case review is $30 and payment is expected at the time of service.

REMINDERS:

1) All ASAP fees must be paid according to the financial agreement (this applies only if you were granted one) when you enrolled in the program. Please ensure your successful completion by taking care of these payments.

2) You are responsible for immediately informing the ASAP office of any address or telephone changes..

3) The Agreement to Participate form that you sign doing enrollment is an important document, and will use in court if you violate any of the conditions of the agreement. PLEASE READ IT CAREFULLY.

4) Once you have completed all assigned program requirements, you should contact the ASAP office one month prior to your restricted operator's license (ROL) expiration date. – THIS DATE IS LISTED ON YOUR GREEN SHEET – so your completion information can be submitted to DMV.
